The Home from Hospital service

Free service for over-fifties

The Home from Hospital Service is a new service commissioned by the Kent Health NHS Trust, in partnership with Age UK Canterbury.

The aim is to give help and support to people who have been discharged from hospital, in order to encourage independent living at home – thus avoiding re-admission to hospital.

Anyone can phone to arrange support for themselves, or they may be referred by a health or social care professional.

Initially one of our friendly Home from Hospital team will visit – either whilst the client is still in hospital – or in intermediate care, or at home. We will then make an assessment of the help that is required. After that an arrangement will be made for up to 12 hours of help over a 4 week period.

The sort of help we can offer is the simple, practical, day-to-day chores, such as light housework, the preparation of light meals and refreshments, help with shopping and the collection of Prescriptions, help with correspondence and phone calls, and of course, emotional support. We can also help to obtain further information, services or equipment as required.

We offer the service to anyone, over the age of 50 returning from hospital. Unfortunately we are only available in the CT1,CT2,CT3 and CT4 postcodes – but within that area nowhere is too remote for us to get to.

The service is available from Mondays to Fridays, 9.00 to 5.00, with an on-call rota at weekends. Our team are friendly and efficient, and have all undergone security checks.
